The solution leverages a customized patient registry along with Sutherland’s SmartHealthConnect tool. Sutherland developed a patient registry for CHN to improve the overall health of CHN’s members. Sutherland’s proprietary SmartHealthConnect tool provides real-time information at the point of care, including risk scores, gaps in care, and admit discharge and transfer (ADT). Analytics are then leveraged to improve health outcomes, increase the quality of patient care, and lower costs.
