ALBANY, N.Y., June 24, 2016 (GLOBE NEWSWIRE) -- House calls are making a comeback thanks to a new local business that is transforming care for thousands of local patients. Landmark Health is providing in-home, concierge-level care to Capital Regions residents with multiple chronic conditions.

Landmark clinicians are on-call 24-7 and guarantee a response time of less than one hour. The team consists of physicians, nurses, care managers, social workers, pharmacists, mental health experts, and dietitians.

"The Landmark team of mobile clinicians acts as the eyes and ears in a patient's home, collaborating with primary care doctors and specialists on individualized care plans. We are providing these services at no out-of-pocket expense to the patient with the ultimate goal of reducing costs by keeping patients out of the hospital." said Landmark CEO, Adam Boehler.

It is well-documented that spending for health care services in the U.S. is concentrated among a small percentage of people with high utilization. According to the National Institute for Health Care Management, just 5 percent of the population is responsible for 50 percent of all health care spending.
Landmark has partnered with two local plans, CDPHP and BlueShield of Northeastern NY, to provide members with a number of services, including:

  • 24/7 access to in-home and telephonic care
  • Triage and crisis intervention
  • Mental health
  • Care coordination
  • Caregiver and patient education
  • Prescription drug management
  • Nutrition coaching

"Working with Landmark Health, we're maximizing the quality of life for our members and their loved ones; we're identifying and addressing health conditions before they become too serious; and we're avoiding costly trips to the emergency room," said CDPHP president and CEO, Dr. John D. Bennett.
"As a former practicing Geriatrician, I am very sensitive as to how we serve our Medicare population. At Blue Shield of Northeastern New York, we are proud to partner with Landmark Health to bring the best possible care to those who need us the most." said Vice President, Regional Executive and Medical Director, Dr. Kirk Panneton.
